The scenario
In partnership with the University of Manchester Institute of the Brain, the dementia Academy, NHS England Greater Manchester and East Lancashire (GMEL) Clinical Network, a piece of work was required to enable understanding of which patient pathways should change to address common issues relating to emergency hospital admissions in dementia.
The objective
To use hospital episode statistics (HES) data to enable understanding of patient hospital service usage in dementia across Greater Manchester and East Lancashire.
The outcome / Results
The data has spearheaded the development of a multidisciplinary, multi-agency integrated dementia pathway tool kit which highlights trigger factors for GPs, that might precipitate hospital admission so that these can be avoided.
Patients and carers were central to the production of the work along with organisations including Greater Manchester Police, ambulance service and voluntary sector were involved. All 12 CCGs in the area have participated and service transformation is underway. The findings have also been shared with other health economies.
